Damn, smudging means making the face smooth and all so it removed most of the stuff around his face so I'm not entirely sure what you mean by putting blood there @Squanchy

You don't actually have to use smudge. Just get a blood png or blood transparent png. Place it onto the image, change the blending mode a little, erase a little area. Duplicate it and change the mode again, but lower the opacity. Use the style option (shadow, inner shadow.. that thing) to make it more realistic. I believe you can add a bit of inner-shadow for more depth with a darker color.
